I do hereby give and grant to Atlanta Housing (AH), all literary and property rights, title, and interest which I may possess to the print, photo, audio or video photograph(s), recording(s) and transcript(s), collected in this form.

I authorize AH to use and/or refer to my person/name, photo or video image, including the use/publication of personal information provided by me (name, profession, quotable material, etc.) in its newsletters, websites, social media pages, brochures and any of its communication materials, including pamphlet/online/media/video devices. I agree to hold AH harmless from any results or consequences that the use of any type of photographs (including all head shots, full-body shots, or any shot where I am recognizable) or personal information may have, either foreseen or unforeseen.

I understand that I will not be paid, compensated or entitled to any benefit as a result of the use of my name, personal information or photographs of any kind.
